If you've ever been on a dev project of any kind, you know exactly what this diagram is talking about. In preparing a presentation this week, I remembered this image and just had to go scouring the web for it to illustrate what happens when things go off the rails.
As a BA, my favourite image is the empty one... the one missing the documentation. As a former consulting firm employee, my second favourite is the sales-guy reference; the image of the armchair with beams of light emanating from behind. Yup, been asked to deliver that many times. Usually without any documentation... ;)
